Transcription and notes

Washington State Board of Health — Bureau of Vital Statistics, Certificate of Death, Record No. 346. Pierce County, Orchard Hill / Tacoma. Ancestry "Washington, U.S., Death Records, 1883–1960" + maintainer-supplied scan 2026-06-26 ((image held in the family archive)). = catalog [p.1036].

Data from the document: - Full name: Oliver Cooper Ludlow; Male, White, Married - Date of birth: 28 January 1839 [FaG [G.265] gives 27 Jan 1837]; Age 79 - Occupation: Retired Farmer - Birthplace: Indiana - Father: Stephen Ludlow (birthplace "Indiana" [sic — Stephen b. NJ]) - Mother: [?] "Cursey" (birthplace Indiana) — first name illegible; surname/name reads "Cursey" - Date of death: 2 June 1918; cause: old age + senility - Informant: Mrs. Mary Ludlow (the widow, Mary Jane Gard [p.1518]), Orchard Hill - Burial: Tacoma

Note (not on the document): the father "Stephen Ludlow" + mother "Cursey" here = Stephen Decatur Ludlow [p.1520] (1809–1884) × Emiline Eliza Kersey — the "Cursey" reading = Kersey, the actual mother (NOT a garbled recall). Corrected 2026-06-26 — this is NOT our Stephen Ludlow [p.883] (b. NJ, wives Jane Porter/LeeAnn Beall): the cert's "father b. Indiana" + the Ancestry tree [L.144] + Oliver's Shelby Co. residence place him in the Shelby-Co. Stephen Decatur family. (I briefly mis-read this cert as proving our Stephen — the unnamed "Stephen Ludlow" + "Cooper" middle name fooled the attribution; the real L.4 "Oliver" is Oliver Perry [p.1519].)
