Texas standard certificate of death, 1918

Note

THIS CERTIFICATE NAMES HER PARENTS, AND THEY WERE PREVIOUSLY UNKNOWN TO THE CATALOG: father SAMUEL THOMAS LEONARD, born TENNESSEE; mother's maiden name MATTIE GRAVES, born ARKANSAS. It opens a Leonard-Graves family that the catalog had no handle on at all - Rosa Caroline (Leonard) Cooper was a stub until this morning and her maiden surname rested on her son's 1951 death certificate [F.345]. THE INFORMANT IS HER HUSBAND, 'G. W. Cooper', of 'Glenrose, R. 1' - the SAME ADDRESS he gives on his own draft card seven months later [B.130], which corroborates that reading independently and in a second hand. PLACE OF BURIAL: WALNUT SPRINGS - the same town where their son Joe B. was buried at Oak Grove Cemetery in 1951 [F.345], so the family had a burial ground there thirty-three years earlier.

Reading notes

ITEM 16, DATE OF DEATH, IS BLANK. Month, day and year are all empty on the form, confirmed on a 3x crop. THE CERTIFICATE NEVER STATES WHEN SHE DIED. What it gives is item 17, the attending physician's certificate: he attended her from 12 Feb 1918 to 19 Feb 1918, last saw her alive on 19 Feb 1918, and death occurred 'on the date stated above' - which is the blank. ⇒ The 19 February 1918 date this catalog records for her comes from her HUSBAND'S PETITION [A.214], not from here; this certificate is consistent with it and does not assert it. THE NAME AS WRITTEN IS 'Mrs Caroline Cooper' - 'Rosa' does NOT appear on the face of the certificate, though Ancestry indexes it as 'Rosa Caroline Cooper' and offers 'Rosa Caroline Leonard' as an alternate. The date of burial reads 'Feb 2[0?]' - the second digit does not resolve. Item 4, Color or Race, is written 'American'.
